It depends on how noisy. If you're in a dance club, then masked is most likely not the best option. If you're in a loud construction job, then you'll want to increase the volume some. If you're dealing with normal city noises, not necessary.

You can change listening from speakers to headphones as you like. You can use both on the same day as well.

If it is at all possible yes, always do back to back. If that isn't possible, or you find that makes you too tired to work, then split it up as minimally as possible.

You can, but it's better to use one format and stick to it if you have that option. Sometimes, people don't have a choice, and they must do this to get in their loops. It's better than nothing, but generally not as good as sticking to a single format in most cases.
