PTSD is one. Dealing with a major breakup, a death in the family, loss of a pet, loss of a dear friend. Dealing with past traumas. Overcoming fear isn't about emotional healing & pain relief. I don't see how you could think they cover the same territory.
In my personal case the fear of not wanting to experience the pain of an incident usually inhibited my processing of the emotional pain. Once the fear is removed the pain is processed, and rather quickly.
Over the years it appears that the fear of the pain became too much that I didn't want to process it anymore as it just kept coming. Maybe I'm so turned off emotionally at this point that it's actually been a while since I experienced true emotional pain.
