Fantasies are just that... Fantasies. I've had a few girls/women tell me about that kind of fantasy, and others that may seem even "worse" to you. When they know you won't judge, women tend to share those thoughs.
I'm quite sure none would like it to happen in reality. I would not say they have "issues" at all. They just get turned on by thinking about it. No need to make a big deal about it, in my opinion.
